We assess the client, agree goals, advise on the treatment route and identify any equipment, environment or wider-agency needs.
Goals are reviewed regularly. Interim reports and updates can be built into the package of care where required.
Packages are tailored to each instruction, from assessment-only work through to ongoing rehabilitation and multi-disciplinary support.
In 2015 a case manager referred an 8-year-old boy with a diffuse brain injury — hoisted for every transfer, with a plateau reported. Ten years of goal-led, MDT-coordinated rehabilitation later, he walks with one crutch, washes and feeds himself independently, and his BMI has fallen from 30 to 22. Less care dependency.
