Case Managers For Physiotherapy
For Case Managers
We are experienced treating therapists to case management companies and solicitors. We offer the ability to provide both physiotherapy and occupational therapy services with the benefits of having very close communication, streamlined methods of working, joined up care as well as the benefit of dealing with one provider. We also offer services individually.
Working under instruction based on the needs of each individual client we can offer a wide range of services.
At our initial visit, we will perform a comprehensive assessment. This is used to highlight any areas which may require input allowing us to formulate a treatment plan for your child.
Where appropriate, we use standardised assessment tools as an aid to benchmark ability and track progress. Where standardised measures don’t fit, we use goal setting tools such as the Goal Attainment Scale (GAS).
Throughout the assessment process we will engage with the client about their worries and put their goals at the forefront of our treatment.
Detailed reports are completed following assessment along with recommendations for future therapy.
This may include advice on referrals onto other agencies as well as recommending equipment. We write detailed justifications for our recommendations and include quotes for any equipment proposed.
Further to detailed assessment reports, we write care packages or treatment programmes for the client which can be used at home, school or with carers to so therapy is transferred into day to day life or education.
At each stage goals are set and reviewed at regular intervals. Interim reports & updates can be included into the client’s package of care.
